The support you provide will be crucial in helping people stabilise and sustain their accommodation and ultimately transition away from homelessness. This is a highly rewarding and diverse role with a mixture of outreach, tenancy and crisis case management support

 Your responsibilities: 

  • Engage with persons sleeping rough and/or those people in danger of becoming homeless.
  • Undertake home visits with clients located in temporary, transitional or permanent accommodation.
  • Refer clients to appropriate external services and link to life skill and educational programs
  • Create collaborative partnerships with other key stakeholders and services.
  • Offer client support at all times with a respectful, non-judgmental and inclusive manner, upholding and respecting the rights of clients at all times while maintaining client confidentiality.
  • Develop case plans with the clients, provide ongoing support to achieve the case plan and monitor the case plan.
  • Attend to the primary needs of clients and maintain client records/case notes, both electronic and handwritten.
  • Meeting service targets as required by the manager, Society and/or external agencies

To be successful, you will need: 

  • Engage with persons sleeping rough and/or those people in danger of becoming homeless.
  • Create collaborative partnerships with other key stakeholders and services.
  • Ability to provide respectful, non-judgmental, and inclusive support upholding and respecting the rights and privacy of clients at all times.
  • Develop case plans with the clients, provide ongoing support to achieve the case plan, and monitor the case plan.
  • Attend to the primary needs of clients and maintain client records/case notes, both electronic and handwritten.
  • Meeting service targets as required by the organisation and/or external agencies
  • Excellent written and oral communication skills & sound computer skills
  • NSW driver licence.
