We have an exciting Coordinator position that has become vacant covering our Inner West Sydney Home Care Services, based in our Ashfield office.

This is a permanent full-time position, 76 hours per fortnight, Monday to Friday, 8.30 to 4.40. 

The main focus of the role is to manage a portfolio of community-based customers who have been allocated home-care packages or Commonwealth Home Support Program Services. The main responsibilities include identifying and mitigating risks, developing tailored care plans, managing budgets and coordinating care services to enhance our customers' quality of life and maintain their independence. You will be inspired and motivated by making a positive difference to the families and achieving ultimate customer satisfaction.

This will be achieved by working with the broader Community Team to achieve PAC’s Mission and Strategic Plan in a manner consistent with PAC’s Values.
