• Providing a person-centred care to help clients achieve their goals and aspirations.
• Contribute information for the development of client’s support plans.
• Provide support to clients with personal care, living skills, transport, social activities, medication, and other support as required.
• Keep client records up to date daily.
• Develop and maintain professional relationships and networks with clients, families, caregivers and key stakeholders.

Requirements:

• Completed Cert 3 or 4 in disability, Individual support, Aged Care, Nursing (or in the last 3 months of qualification)
• Access to your own vehicle ( License and Comprehensive Insurance)
• At least 6 months experience in disability, aged care or nursing sector (preferred but not required)
• Proof of Vaccination ( COVID 19)
• Working with Children's Check (or willing to apply)
• NDIS Worker Screening Check (or willing to apply)
